Environment
Step 1:
The conditions are often overcrowded and unsanitary. The dogs typically lack food, water, exercise, grooming or veterinary care. In addition, they are generally unsocialized and aren't supplied with toys or treats.
Health

Puppies that come from puppy mills are often in poor health and have genetic defects. The Humane Society of the United States notes, "Because puppies from puppy mills are more likely to have health problems due to poor care, many consumers are faced with significant veterinary bills or even the death of their puppy soon after purchase."

Breeding
\Female dogs have very little time to recover between litters. Females are often killed when they can no longer reproduce.

Outcome  
The puppies are often sold to pet stores. Puppies that have defects will not survive because they are not considered to be profitable.
